- Online: Download New Patient Packet here
- Mail packet back to: Ko-Kwel Wellness Center, Attn: Registration 2401 River Road, Suite 101, Eugene, OR 97404
- In Person: Pick up a New Patient Packet from our registration desk in person.
- Phone: Call our office to request the New Patient Packet by mail.
- Online: Download New Patient Packet here
- In some cases, applicants will be encouraged to apply for the Oregon Health Plan (OHP).
- Assistance is available for this process.
- To follow up on your application, call Registration at (541) 916-7025.
- Choose your provider/service and make an appointment for care.

Next steps
Be an active team player. Talk with your KWC health team about health questions, health challenges, and any other health care professionals who are caring for you.
Your role as a patient
- Communicate closely with KWC
- Keep KWC current with your medications, immunizations, allergies, conditions, tests, consultations, and hospitalizations
- Advise KWC of any changes in your medical and/or dental history
- Inform and authorize other providers to coordinate with KWC for collaborative care
- Participate with your team to make decisions about your health
- Follow treatment plans and manage your self-care
- Speak up and ask questions
